Flu Immunizations
Bismarck-Burleigh Public Health offers flu immunizations.  Below you will find the forms required for vaccination.  If you have not been to Public Health in the last 12 months, please complete both forms. 

To schedule an appointment for an immunization, or for more information visit flu vaccination, or contact us at (701) 355-1540. 

Flu Vaccination Forms

You may print and complete the forms ahead of time and bring them to a scheduled flu clinic to help speed you through the process.  Please remember to bring all of your insurance cards with you to the clinic.

Bismarck-Burleigh Public Health accepts the following insurance plans:
  • Medicaid
  • Medicare Part B
  • Blue Cross Blue Shield
  • Medica
  • Select Care

If you do not have coverage through the above insurance carriers, you will be required to pay for the vaccination at time of service.  Our fees are as follows:
Flu Shot            $30.00
Flu Mist             $35.00
High Dose         $40.00

Be a part of APHA's Flu Near You Challenge

So how does it work? APHA’s Get Ready Campaign has teamed up with HealthMap to help track flu cases through a new online flu surveillance tool, Flu Near You.

Anyone can use the tool to report their flu symptoms, which will help map influenza. Please join Bismarck-Burleigh Public Health in support of the Get Ready Campaign and Flu Near You Challenge by registering your information.

Bismarck and Burleigh County Community Health Assessment

Community Health Themes and Strengths Assessment

PURPOSE:
The focus of this survey is to learn about our community’s assets, concerns, and services provided, as well as identify any gaps. The survey was designed for public input from the city of Bismarck and Burleigh County residents to learn about our community’s health needs. In this survey “Community” means where you live and interact with people.  The opinion survey opened February 1, 2012 and closed March 1, 2012.

Local Public Health Systems Assessment

PURPOSE:  
The local public health systems assessment (LPHSA) measures the capacity of the organizations and entities of the local public health system.  The LPHSA answers the question "What are the components, activities, competencies, and capacities of our local public health system?"

Community Health Status Assessment

PURPOSE:
The Community Health Status Assessment is data compiled, within our community, that identifies priority community health issues, as well as, quality of life issues. The data will help our community answer the following questions:

"How healthy are our residents?" and "What does the health status of our community look like compared to other communities in our state?"

Forces of Change Assessment

PURPOSE:  
Go! Bismarck Mandan, our local coalition working to build a healthier community, used the CHANGE tool to understand feedback on actions that should be developed and implemented to transform communities into those that support healthy living.

CHANGE provides a snapshot of policy, systems and environments over five modules: Physical Activity, Nutrition, Tobacco, Chronic Disease Management, and Leadership.  With CHANGE data as a guide, you can map out a course for health improvement in your community with specific, targeted action.  As a result, it helps communities to assess potential approaches that are innovative as well as approaches that show strong evidence of effectiveness.
